    Abstract 2011年3月11日に発生した東日本大震災は甚大な被害をもたらした。本稿では発災直後から岩手県災害対策本部の医療班長として対応に当たった私の経験を元に、災害医療に関連するいくつかの教訓を抽出したい。まず第一に、災害は常に想定外でも対応をしなければならないことが重要である。そのためには緊急事態における標準化と指揮調整のシステムが必要である。それと同時に、混乱を極める被災地では住民が全てにおいて公的機関に頼れる状況ではなかったため、自助・共助への理解と育成が重要であることが伺える。また被災地の支援者たちは被災者でもあることを考え、自分たちの身も守ることを心得る必要がある。こうした教訓は、来る首都直下型巨大地震や南海トラフ巨大地震へ備えるために、東日本大震災の教訓をつなげていく必要がある。
